As a caregiver should I have to pay clients household bills? I don't even get paid for my services.

Do you live in his home? Even if you do, you should not have to pay his bills. He'd have to pay them if you weren't there. Since you aren't being paid, how could you be expected to pay for anything. You caregiving is free, what more do they want.
