Grandma fell fractured her pelvis, went to nursing home for rehabilitation, fell there & broke her femur. Am I about to lose her?
Had surgery and went back to the nursing home. That's been 2 weeks since and couple days ago passed out, doctor said its her enzymes around her heart. Am I about to lose her?
Tell her how much you love her, and share fun memories with her. Play music she loves, sing to her. And give her milkshakes or other treats you two can share.

Cardiac (heart-related) enzymes are released when a person has had a heart attack or injury to the heart. The level of enzymes can sometimes be an indication of how much damage there is.
You want to ask her doctor what the outlook is, if she is expected to recover and what to expect her quality of life to be.

I have a practice now of recalling every second of those countless falls, ambulance rides, hospitalizations, etc., and turning those seconds into a hundred rays of love and support to all of my fellow brother and sister caregivers❤️.
