Get as much verifiable documentation as you can, copy of the document, and any medical documentation attesting to his inability to comprehend post-stroke, then consider contacting any or all of the following:
Local PD or Sheriff's Department; County or State Elder Law agency (or something similar), Area Agency on Aging (although they would probably refer you to a law enforcement agency.
Has the authority been used in any transactions? Were there any witnesses to the forgery? How did you learn about it?
You'll need as much information, verifiable and documentable, as possible.
