My father who is 87 recently was released from the hospital with a UTI and pneumonia after fainting. What is the best home care plan?

He declines palliative care.

Before he was released from the hospital, you should have met with a social worker or discharge planner. They would have set up home care for you and ordered any home medical supplies or equipment he needed. I would call his doctor’s office and ask for a redo on his discharge and tell them what you need.

Just to clarify, the hospital sent him home with a UTI and pneumonia? Or is that why he was admitted in the first place and he came home well? I'm hoping they didn't send him home sick...thanks for the extra info.

Was home health care ordered? Usually, that entails a visiting nurse and some therapy like OT and PT for a few weeks. Perhaps also a bath aid?

Are you his full time caregiver? Was inpatient rehab not offered?
