I received a letter informing me that I would need to show proof of Medicaid eligibility but I am unsure about how to get that. Do I need to to get a letter or a form?
With both you should have received a letter saying that you have been accepted and day your coveraged covered. You should receive a card saying you are covered by Medicaid. These would be your proof. If this is for taxes, Medicaid sends proof of coverage. My nephew just received his in NJ. Like said call your local office.
Once your eligibility has been determined, you should receive a letter of eligibility. If you have not, the Medicaid information line in NY is (800) 541-2831. Hope it works out for you.
