At 19 you need to begin building your future. Call the Area Agency on Aging for support, they can help get grandma into a facility so she gets the care she needs and you get the life you deserve.
Medicare does not pay for assisted living or nursing home. Medicaid will. You would need to apply for her if she has no assets and she is incapacitated.
Call Council on Aging in your area for help. Contact a social worker from her doctor or the hospital that she uses and ask them for suggestions too.
Why are you taking on this responsibility? You need to be concerned about your own future. Where are her children? Do you live with her? Tell us more please.
