When is time for a hospice facility?

Mom has been on hospice care in her home for approx 9 months. She has qualified at least 3 or 4 times for continued services. She is on the cusp of diminished capabilities in her home. She has end stage CHF and her breathing is getting worse. She has decided against a new pace maker and the battery in her 10 yr old pace maker is running out of steam. While she can still do some things for herself, it is taking her longer and longer to get it done and she depends more on her CNA from hospice. Some days, I’m hesitant to leave her alone. She knows about the beautiful facility we have in the area for hospice and she is somewhat agreeable to go there when the time is right, but it will be emotional as this would mean finally leaving her home of many years. Everything will be so final for her.

When do I put her on the list? I am going to talk to the social worker from hospice this week in front of her, and I hope to open this up more in front of my mother. She is almost 94 yrs old and has slowly been going downhill throughout this year.

When will I know it’s time for sure? I would feel better if she was at the facility now, as I can’t be with her 24/7 and things are only getting worse - not better. TY.
