Mom keeps having bowel movements she says are very painful, screaming and crying. Her bowel movements are mainly liquid bc that is all she eats is liquid and soft foods. She also puts her hand down her diaper and gets feces all over her hand and the bed. Please help me somebody. How do I deal with this issue? I’m so tired of changing her diapers and bed. It seems like she defecats three or four times a day. Is that normal? If I try to leave her diaper on for a little while but then she removes it herself. She is a very agitated patient and gets combative sometimes. The doctor has her on a low dose of Respiradone 1 pill in the morning and 2 at night. I’m not sure if it’s really helping that much. The only pain meds she is on is Tylenol and Ibuprofen alternately.

can she get onto a portable commode? If so, put her on it 15 minutes after eating.
Do try to eliminate the milk products. That could be a cause. Perhaps a protein drink, with almond milk, fresh fruit, and a bit of ice blended.
It can be incredibly painful to pass even loose stool when suffering from IBS, the cramping can be excruciating. I have been brought to tears with the pain as it travels through my small intestines. I describe it as someone taking a garden fork to my intestines and twirling it.
It is also not unusual to have multiple rounds of "Tummy Trouble" in a day. For me anything up to 5 BM's a day is my "normal".
Have you asked her doctor about a referral to a Dietician? It may be challenging to get into an appointment, right now, but you should be able to get a phone consult.
Ibuprofen can trigger tummy upset. Perhaps try not giving it to her for a couple days and see if things settle down.
My first thought is if she is having problems digesting dairy and if the soft foods and beverages you are giving her are primarily dairy that may be the culprit right there. People can develop dairy intolerance at any age.

jjcares, are you using tab type briefs or pull up type?
I used the tab type on my Husband and as odd as it sounds I would put them on "backwards" so the pull tab was to the back so it was more difficult for him to pull at the tab but it was also easier for me to get them on tighter so they would not slip. I also used briefs, yes regular boxer briefs over the tab brief this made it impossible for him to pull at them and they did not slip off.
Make sure she does not have a blockage.
Also use a barrier cream after cleaning her. Urine and fecal matter can be harsh on the skin and cause discomfort.
