First they don't "take over" her Medicare. Medicare is a health insurance that people, who paid into it while they worked, are entitled to when they turn 65. Medicare pays for Hospice. Works like any other health insurance. Hospice bills Medicare and Medicare pays them.
Social Security is only involved when the person needs a Nursing Home and doesn't have the finances to pay for it. In this instance, Medicaid is applied for. SS and any pension are used to offset the cost of the NH with Medicaid paying their share.
