If they just give you money without a legal contract and no one is paying taxes on it, it could endanger their current or future Medicaid qualification. That would be a mess to fix. Good luck!
States do not usually pay for personal assistants. But, grandparents could pay you. Be very careful with that, the income could very likely be taxable and needs to be reported by you as income so you do not put Medicaid in jeopardy.
