My friendvwas in a wheelchair from losing a leg. The County Dept of Disabilities loaned him a ramp that they installed. When he passed, it went back to them.
There are volunteer groups that build great ramps, check around. Or hire a contractor. Or consider renting a ramp from a ramp company. They are usually metal and do not permanently alter the home.
Hire a contractor to build you one to local codes. This is not provided by the government for free. We had a metal one that was $280.00; would carry up to 650 lbs
I'd take a little drive around my neighborhood and see how many homes have ramps (a surprising number!) and actually check to see how they look (some are obviously family-rigged and pretty ugly) some are seamless works of art. I guess a lot has to do as to whether the family feels the ramp will be needed for a short term or long.
Find out who built the ramp you think is safest. I know some neighbors kind of threw one together for their wheelchair bound mother and it was just 2x4s and plywood and not sealed against the elements--fell apart pretty quickly. She was over 300lbs and the thing just could take the weight.
There are a couple homes in my 'hood that have metal ramps that must be 'temporary', but look very strong. They are installed quickly and while yes, they look industrial, they also look strong.
You need to hire a carpenter. Ask around for suggestions. Are there ramps near you. Ask the owner of the house.Check with your Township code office to see what is allowed.

I checked before I posted, and immediately saw one with a weight limit of 800 pounds. You might want to take a look too.
