Am I allowed to get a copy of the FL2? I need to transfer my mother to another LTC. It takes too long to get one sent from existing LTC.

I am my mother's POA. She is in a LTC facility an hour an a half from my home. My 86 year old father lives with me and is physically frail requiring a rollator. He has many heart issues and traveling this distance and back to see my mother is very difficult for him. I need to move her to a closer facility. She is Medicaid approved. The facility she is in does not want to send her FL2 form to facilities that I ask them too. How can I get around this? As her POA am I allowed to get a copy of the FL2 and forward to potential facilities myself? Or, is there a patient advocate that can help me transfer her to a closer facility?

Get copies of all and any paperwork you need. Seems you'll need to handle the paperwork yourself. You can do this!

This is a form the doctor filled out, correct. Does he not have a copy in her file. Drs usually keep this kind of info.

I actually had to stand over the knot head that was supposed to send the paperwork to the new facility. I got the new facility on the phone and confirmed while I stood behind the person sending the paperwork that they were receiving everything and that they had everything that they needed. I was the bad guy but, I couldn't believe how easily they lied to my face, I was told that they had sent the paperwork like 4 times. Amazing how it was actually received when I stood there ensuring that it was being done. You will probably have to do the same thing. I think that it does something to their rating when people leave.

These people act like bottom feeders when you want to transfer someone from their facility.

I would contact the ombudsman and ask that they help you file complaints against this facility. There is zero excuse for not providing the paperwork to get mom transferred.

I'm absolutely not qualified to answer therefore may be all wrong, but wouldn't the new facility have to do their own intake form?

They do their own forms but, they want the roecords from the current facility. This will show meds, hospitalizations, level of care, etc.
