I am my mother's POA. She is in a LTC facility an hour an a half from my home. My 86 year old father lives with me and is physically frail requiring a rollator. He has many heart issues and traveling this distance and back to see my mother is very difficult for him. I need to move her to a closer facility. She is Medicaid approved. The facility she is in does not want to send her FL2 form to facilities that I ask them too. How can I get around this? As her POA am I allowed to get a copy of the FL2 and forward to potential facilities myself? Or, is there a patient advocate that can help me transfer her to a closer facility?

These people act like bottom feeders when you want to transfer someone from their facility.
I would contact the ombudsman and ask that they help you file complaints against this facility. There is zero excuse for not providing the paperwork to get mom transferred.
