In my area there is no such thing as a Hospice Center. If you go on Hospice its usually in home with family doing most of the care. My Mom was already in LTC so Hospice came in there. When Hospice was not there, Mom still got care from the facility staff. Hospice can be done in a Assisted Living too. But Medicare does not pay the cost of the facility. It only pays for the care. Providing a nurse and aide. Supplying incontinence supplies and prescriptions and other things.
Her doctor will need to make the referral for hospice, maybe they work with a good one.
I would speak with her care providers, her social worker, her doctors, these are the people that know what is going on with her and hopefully, know the best resource for her needs.
