What form do you use to correct your Federal taxes?

If you are correcting your personal income tax return it is Form 1040X.

Consult a CPA or the IRS themselves. Do know you will be on hold for a long time. They have an excellent web site for you to explore if you are more capable than I, and you may be able to access your forms directly from that site.
