Home Care Plan. Advice?

Mother-in-law will be home in 2 weeks from Physical Rehap. Chipped Hip bone, no surgery. Husband is co-guardian with his sister, and they are in midst of court battle. We have been living with mom for over 2 yrs helping her. She is 98 and still cares for herself and her 2 cats. Had mild dementia that has become a bit worse since the court situation has caused undue stress. I need to write a home care plan for the court to have them approve her coming home. Haven't had much luck with this judge so I really do not want to mess this up. I know what to do but putting in on paper in a plan form has me somewhat confused. Been trying to find a prewritten plan to grasp what I need to write
