This a message you should send to your primary doctor, not this forum. You say you have osteoarthritis in your spine, mainly the lower back. There is no cure for an aging spine. But you might benefit from physical therapy and an occasional pain pill. This is something your primary care doctor can prescribe for you.
Pangel, Barb is correct. Oak Street has clinics here in Michigan. My own experience with them is the solicitations they sent in the mail.
I think you might have better and quicker response and luck with a group of orthopedic surgeons, as it seems as though your pain is orthopedically related.
How long has this been going on? If it's recent, can you think of something that might have caused the pain? Shoveling snow, for example? If it's of longer duration, have you seen any other orthopedic doctors?

Is there any reason that you think that they wouldn't try to discover the source of your pain?
I would advise calling 911 and getting diagnosis and treatment at a hospital if you can't conveniently get to Oak Street on your own.
