My DH aunt had a group that came to her referred by her Home Health Agency. You could call such groups in your area, also call NHs and ask if they have visiting doctors or podiatrist offices in your area.

Depending upon where you live, there may be podiatrist care available paid for by Medicare, and you also may be able to arrange for services provided to her where she is living, but you will have to research this yourself.
If she is receiving Medicare benefits now, see if her paperwork contains a co tact number, and give them a call.
Because you are already dealing with someone with severely compromised circulation, because failure to heal a tiny nick can mean sepsis and death, you should see only an MD for footcare imho. I was a nurse. Can't tell you how many amputations started with nothing. My own brother died of ignoring a small cut on his shin (sepsis). Don't risk anything but professional care. Ask your MD for referral.
