Does anyone have any ideas, reasons and/or solutions as to why my mother-in-law (relatively recently) started making a constant groan noise?

You do not give any info on your MIL. You mention you are caring for your mother living in Independent Living.
What is your MIL condition?
Does she have dementia of any type?
Is she verbal? have you asked her what is wrong?
If there is no answer or you can not figure it out AFTER anything medical has been eliminated as a problem then you go by "clues"
Does her facial expression indicate pain or discomfort.
If no then the noise is just that noise.
Noise can be a "self soothing" tool she is using to comfort herself. While annoying to you and others it is something that you need not worry about. Just be glad it is moaning and not another louder more obnoxious noise.
My Husband moaned for 10 years. Sometimes quietly, other times a bit louder, yes it drove me a bit batty sometimes. And oddly there are times I miss it.
