Echoing MACinCT, really if they are currently in a NH on that States LTC Medicaid program it will be very very difficult to do. Hell it’s a difficult ballet 🩰 to move from 1 NH to another in the same city or state.
but whatever the financials, as a first step please get a needs assessment done on the elder. The current NH should know of a team (tends to be a RN & a SW) who do theses and you take their report along with last 6 months of their NH health chart to any of theNH you are considering and let them go over them so they (they not you) determine if they can meet the level of care your family member needs.
The NH will know of transportation companies as well…. And this will be expensive. Just how $$$$ depends on what her in transit care needs are and how far as they will NOT drive full 24/7. There will b stops and all of this you pay for. MediCARE will not pay for it. If you have a ton of $ you can do private medi flight.
