HeyPronker. I live in California, and our county’s senior bus system does this. It’s 4.50 each way with fee waiver for low income. If you can get to the appt from the curb, far better than a 130 dollar round trip.
Pronker, I recall doing some research a few months ago on this topic, and did find that Medicare would pay under certain conditions, but I can't find the link in my database, and don't remember for certain.
Is there a Senior Center in your area? Some of the more inclusive and exhaustive ones do have their own small buses (point to point transportation) leased from a transit agency, and have a schedule of pickup for medical transport.
Our local transit agency also offers point to point service.
Your local AAA might be able to offer some insights.
A few years ago I needed transit for post cataract surgery check-up, and found after research that one of the local ambulance companies also offers private point to point service.
I hope you can find something suitable, and reasonable.

Thank you for replying and I'll keep the hints in mind. It may be I'm needing to specify Medicaid *Medi-Cal* in California, that is, and they will answer this question. I keep lamely forgetting to call Aging Council phone number, argh! And now it's the weekend. :/ The trips are about $40 each so it's getting expensive. I'm just chicken to return to the bus system which I used before the operation because the Uber/Lyft trips are bumpy enough and it hurts to go over bumps and potholes or even sway very much. :(
