My 96 yr old mom has lived with us for 4 years. This last year her dementia has really progressed. We’ve decided for many reasons to move mom into a smaller assisted living facility. We have been talking to her about the move, but she doesn’t remember. Hoping you could recommend or suggest anything that may make this transition as smooth as possible for mom. She was such an active, vibrant women all her life and to see what this disease has done in a short time is heartbreaking.

It can be a very 'weepy' situation as she will have to make some accommodations and doesn't have the skillset to do that anymore. Be patient and kind and make her transition as peaceful as possible.
I helped a client move to AL and we took WEEKS of going to lunch there, moving in picture, small pieces of furniture and such to help he acclimate. She was phenomenally wealthy and that sure didn't hurt. Money bought a lot of nice stuff and paid for my time, which she really didn't need. I know for a fact she had a lot more visitors at the IL place than she did at home. It truly was a win-win for this person.
I also was able to rearrange the furniture so that Dad's bedroom was the same as what he had at home. That way, if Dad awoke at night, he didn't become scared about where he was. Dad loved nightlights, so I placed those.

Have friends and family send lots of cards and postcards.
