You should be able to do this online at www.socialsecurity.gov. Dealing with them on the phone can be difficult due to the workers not knowing enough to help. It's pretty straightforward on the website.
BarbaraMom, can you give us more information to help us decide what type of answer to give. Such as why is your Mom currently living in a Nursing Home? Is she on Medicaid [which is different from Medicare]? What are your Mom's health issues?
Are you ready to do all the work of 3 shifts that were needed in a Nursing Home? Will you have caregivers coming in? Is the room where Mom be set up like that of a nursing home?
