If she cannot pay you, if she feels like paying you, your other resources are to ask your local social services. If she can pay you, get a contract and keep good records so as not to have problems with Medicaid. You should also pay taxes on that regular income that you make. If she is only Medicaid eligible for in home care, then it is more complex if the State allows it. At home services is barely part time hours and low rate of pay. If you need a break if you are 24/7, you should call your local social services to see if she is eligible for a stipend for you to hire care or send her to adult day care.
If MiL has funds, she can pay you. You should get a contract, perhaps set up by a lawyer if you do this. Make sure you have SS and Medicare taxes taken out so that you will have adequate credits to be able to claim Social Security and Medicare when you are elderly.
