Go to the Dept of Health and Human Services website for your county. Most will have a category for Elder Services of some sort. Contact a social worker and explain your predicament. Go from there.

What you need to do is contact a state social worker to help you or have your grandmother's physician send a referral to one. You tell the SW you are unable to adequately and safely care for her anymore. They should then start the process of helping you find nursing home placement for her. They will help get her started with the Medi Cal application. If that takes too long, and you are in more desperate straits, you need to find a reason to send her to the local hospital (via ambulance is usually best) and ask them to hold her as a social admit, refuse to take her home, and have them help find a facility for her. This route is not easy and is frowned upon by some hospital staff, but in reality, there is often no other solution for those in similar situations to that which you are in. Try the social worker first.

When she does go to a facility all of her monthly income will go to that facility save a small amount left over for her personal needs.

You are POA for your Grandmother? If not, as the caregiver you can become her guardian, and you will then place her in a nursing home. She will certainly require medicaid, and you will, as her guardian, be able to apply for this for her. The funds will go directly to the nursing home, as will her SS.
