Medicare should to an extent depending on your state.

You say you're a daughter seeking to bring him home, out of the NH.

Is your Father currently a Medicaid recipient?

Why do you wish to bring him home? Providing in-home, hands-on care will be very physically demanding, 24/7.

Perhaps he is ready for hospice?

Notice at this link MA in MD will pay for *some* in-home care. It may not be enough. If your Dad qualifies. Whereas, if he is in LTC MA (+ his SS) will pay for all of it, again if he qualifies.
