Who can do cognitive testing for someone who has dementia? Must satisfy both long-term care insurer and possibly Social Security disability.

Already have primary care doctor's letter which works for health care POA and financial POA.

If you are in the USA, the Alzheimers Foundation of America does screenings via phone, if the person is able to talk on the phone. Otherwise, a neurologist or neuropsychologist.

If disability is for wife, it cannot be gotten once 65. She can collect regular SS at 62 but get less than if she waited till age 67.

Doctor needs to refer you to a Neurologist for testing .

I would get the written opinion of two doctors and copies of any testing.
One MD should be neuro-psych eval.
