55 year old is currently in physical rehab after being in the hospital for 3 weeks. He currently has cirrhosis of the liver and a very weak heart. The physical rehab will look to discharge him soon and the alcoholic rehabs will not take him because of his on going medical issues. He also gets confused at times because of HE. (Hepatic encephalopathy). It does not sound like he can be left alone but has no place to live. Anyone know of any social services that would help a person in this condition. He has insurance, although not the best, and has some money to pay for rent if needed.

Get a hold of a social worker see what your options are as far as nursing homes, group home, hospice etc.
You need to talk to a SW and explain that you or this other person is homeless and has no one to care for them. Sorry, but looks like he may be dying. Maybe transferring him over to the NH section and starting the Medicaid application would be a good idea. This is an "unsafe discharge". Hospice should be brought in.

Adult protective services should be contacted and perhaps now a good time to contact any social workers you have access to. You need to keep mentioning "unsafe discharge" wherever he is placed, because if he is eventually being sent home alone it will be unsafe on the face of it.
I would caution against taking on POA or guardianship as it is of course quite impossible with an alcoholic.
He may need placement. It may be time to accept palliative care which will be able to do the lactulose tx. and so on, and perhaps to consider hospice.
The time is either her or coming when he will have to be in 24/7 care.
I am so sorry. I hope if you are close that you are attending Al-Anon. There is nothing like it for SUPPORT and for knowledge and resources help.
