Looking for help with past due income tax preparation. Cannot get to see my own tax preparer due to a walking disability.

I am 84 years old, have walking disablement and am three years behind in income tax preparation although I have requested an extension each year and have paid an estimated amount. I am not looking for a free preparation but rather a person who does income tax preparation in the home. Can you guide me to such a person? My income is 100 k.
