As I said, Medicare gives 5 star ratings to hellholes I wouldn't put an animal in. It's very important to personally check these places out yourself and not rely on "Medicare ratings" which are often a joke
Every facility will have a number that you can use to call and file a complaint. It is usually the Ombudsman's Office. Sometimes there is a State Number that you can call. Depending on the type of complaint you can also file complaints with the local Health Department, Fire Department, Building Code Office or the State Health Department.
Just leave them a scathing review on Yelp and Google......which is what I did after getting mom into a FIVE STAR MEDICARE RATED SNF for rehab that I wouldn't put my worst enemy into. After I transferred her OUT of there, I wrote the reviews which got lots of views and hopefully, prevented folks from putting their loved ones in there. I think Medicare is in cahoots with many of these places, personally.
Otherwise, your states Department of Public Health is a good place to file a complaint.
