Mom was living in an apartment walking distance from my home. 24 hour caregivers due to dementia. Sometime Saturday morning she had a stroke. Can't move left side. Now in hospital, then probably to rehab. Neurologist said not likely back to apartment. So my question is this, for long term care with dementia and stroke do I find a nursing home or is there something else? She is 86.

Talk with the hospital social worker and see if she can make a recommendation for a rehab place with a good reputation that also has long term care options such as nursing home and/or memory care. They help with these transitions often and can advise of good options

I'm so sorry this is going on!

It's probably too soon to tell what level of care your mom will need after rehab.

Make sure to stay in close touch with her therapists (OT, PT, Speech) and with the SW at the rehab facility; they can help you determine what level of care mom needs, going forward.

If you have a choice of rehab settings, is there one nearby that also has a continuing care campus (Assisted Living, Skilled Nursing Care and Memory Care) so that mom wouldn't have to change facilities?
