"Who do I contact to get her there?"

You need a medical transport van. You can find companies if you do a web search. They will give you an estimate. Not sure if Medicare (or any other supplemental insurance) covers this as I'm not on it yet. Others will know.

Medicare may cover part but not the whole cost.
Some companies won't accept it, and will request to be paid in full up front or no transportation.

Can you not just set up a Zoom meeting with said doctor? All of them are doing that now since Covid, and with your mothers mobility issues that shouldn't be a problem. Now someone may need to be with her to get her on Zoom, and perhaps share with the doctor exactly what's going on, if you don't think your mother will be forth coming with her issues.

You need to tell the doctor that mom needs to be assessed at home. Let the send a PA or NP out to see her, or find an eldercare do tor who visits the homebound.

Often the Area Agency on Aging can help with this.

My mother always had to pay out of pocket when I'd call a van to get her to various locations with her wheelchair. Google 'wheelchair accessible taxi service near me' to find a ride for your mother and ask about their rates up front.

Office of Aging usually has a Senior bus thats wheelchair excessable.

Look for non emergency transportation services. I had a company called Medtrex bring my mom home from the hospital for $50.

My mother was bedridden and i needed to get home from an er visit, i called my local fire department and they carried her back upstairs to her bed on second floor. They were extremely nice, I was very grateful.

My doctor did this over a remote zoom meeting via cell phone at the house. Mom did not have to leave the house. Then he wrote the order. You will have to call the doctor's office and request a remote appointment since your Mom can't leave the house.
