I. How We Work in Washington. Based on your preferences, we provide you with information about one or more of our contracted senior living providers ("Participating Communities") and provide your Senior Living Care Information to Participating Communities. The Participating Communities may contact you directly regarding their services. APFM does not endorse or recommend any provider. It is your sole responsibility to select the appropriate care for yourself or your loved one. We work with both you and the Participating Communities in your search. We do not permit our Advisors to have an ownership interest in Participating Communities.
II. How We Are Paid. We do not charge you any fee – we are paid by the Participating Communities. Some Participating Communities pay us a percentage of the first month's standard rate for the rent and care services you select. We invoice these fees after the senior moves in.
III. When We Tour. APFM tours certain Participating Communities in Washington (typically more in metropolitan areas than in rural areas.) During the 12 month period prior to December 31, 2017, we toured 86.2% of Participating Communities with capacity for 20 or more residents.
IV. No Obligation or Commitment. You have no obligation to use or to continue to use our services. Because you pay no fee to us, you will never need to ask for a refund.
V. Complaints. Please contact our Family Feedback Line at (866) 584-7340 or ConsumerFeedback@aplaceformom.com to report any complaint. Consumers have many avenues to address a dispute with any referral service company, including the right to file a complaint with the Attorney General's office at: Consumer Protection Division, 800 5th Avenue, Ste. 2000, Seattle, 98104 or 800-551-4636.
VI. No Waiver of Your Rights. APFM does not (and may not) require or even ask consumers seeking senior housing or care services in Washington State to sign waivers of liability for losses of personal property or injury or to sign waivers of any rights established under law.I agree that: A.I authorize A Place For Mom ("APFM") to collect certain personal and contact detail information, as well as relevant health care information about me or from me about the senior family member or relative I am assisting ("Senior Living Care Information"). My father has Medicaid, Medicare and received SSI and EBT Food. He has no saving and on Dialysis for 7+year. The social worker asked us to prepare for his final day. Is there have any help from county that we could apply. Thanks, Helen
States or counties have methods to pay for the burial or cremation of a person who did not have the means to arrange that for themselves. Call or search on line in your area for something like burials for the indigent or contact your local coroners office. Some medical schools may take a body donation and then eventually pay for the final disposition of the body after it has been used by students learning medicine. The coroners office or a hospice outfit may be able to make suggestions. The family may also wish to make their own contributions to his final disposition. Look now into your options so it's not as great a shock as it will be later.
Aw, phooey...first of all, so sorry you and Dad are going through this, but second of all - why didn't the social worker give you a few leads? One thing you are allowed to do is get a life insurance policy that is really a burial policy that you pay into. But I would not know of specifics at all, or what provision would be made by the county or the state if there is no time for saving up for that...and I hope she didn't mean she thought his time was very limited...
Social Security provides a small death benefit, and if he was a Vet, the VA will provide a headstone, but you would pay to have it installed at the grave. The Social worker can help you with that. If you want a wake at a funeral home, you pay for that but it is not necessary. For that you would appoint pallbearers. Your church will certainly help with a memorial service, so contact your pastor and he will help you pick the readings and the music.. An Obituary must be composed, listing relatives and life achievements, so you need to do that. If dad has a grave plot, find the deed or call the cemetery for the plot number. Figure out what family members need to be notified and make a list. If he had a Will, find that. Find Life Insurance policies, if any. If he owns a house, get a lawyer to find out if you can sell it, or if Medicaid recovers it. Lots to do.
