I haven't received a bill and I'm supposed to have a copay. Wondering if anyone else has had this experience.

Co-pays are usually gotten upfront not billed.

You should have a caseworker. Best thing is to direct your questions to them.

My wife has Indiana home based waivered services. There are no copays for the home care assistance she gets. Her health and drug coverage do have the potential for copays. In her case if I had questions I would contact her case manager regarding home care and medical. For prescription questions I would contact her Medicare part D provider. Your local FSSA office might also be helpful.

From the internet:
"Indiana’s CHOICE program is a statewide program that allows seniors who are at risk of nursing home placement to remain living at home or in the home of their caregiver and receive services, support, and care in those locations. The program serves those who are 60 years of age or older and persons with disabilities of all ages who are eligible due to long term or lifelong limitations. The program is administered through Indiana’s 16 Area Agencies on Aging. The program provides home- and community-based services to assist individuals in maintaining their independence in their own homes or communities for as long as is safely possible.

Really, I think that we can find about what you can find on the internet. You might want to contact them directly. We are only a bunch of caregivers from all the states and not a few countries; we would be the last to know.
Good luck on your research.
