At some point someone has to step up and be the adult. Speak the truth that mom is NOT able to live alone and NOBODY is checking in on her or caring for her daily. Nobody wants her mad, would they prefer her dead?
If you siblings tell the hospital mom is an UNSAFE DISCHARGE, or the rehab facility if they want to send her there to recover, then it'll be the doctors place to have the conversation with mom about leaving her home, not you.
Otherwise you tell her that her physical condition makes it necessary to live fulltime where there are nurse’s .
