I have active POA. I am paying, privately, this staggering bill. PCP has been unhelpful and under-responsive at every turn and I don’t understand why! I would give my right arm to be able to change PCPs but there is not a single provider in my area taking on new Medicare patients.

It is most likely a state rule because your LO is being committed to a locked facility and the LO can no longer make decisions. Some states require 2 docgor signiture.
Also you might want to take him to his appointment with papers in hand so that essentially the doctor can bill insurance for his time.

As for finding new docs, you should be able to go on the insursnce web for a doctor search to find who is accepting. You can also speak to a live representative.

Why isn't there a doctor on the premises of this AL to become hubbys PCP and handle such matters??? One big reason AL was SO great for my parents ( and most other elders) was the in house doctor I switched her to. And when the time came to segue into Memory Care, she wrote the order. As well as all moms prescriptions that were sent into the AL/MC. Mom got to see this PCP every time she wanted, w/o ever leaving the premises.

I am surprised about this.
Generally it is the facility that is having to have the talk with family about moving someone to MC from AL or IL. It is typically a change in care needs or the person begins to "wander" and becomes a danger to him/her self.
Does the facility have a "house doctor" that you could switch to?
If your husband happens to have a trip to the hospital before you can get the PCP to sign off maybe the hospital doctor would be able to do that.

You could "threaten" to begin looking for another facility that will GLADLY take your money to place him in MC. I am sure the facility he is in now would change their tune quickly with the thought of financial loss like that.

They need this to cover their own behinds. I am totally surprised that they don't have needs assessment right onsite that can document the needs and forward to whatever doctor needs to sign off.

Not like this isn't hard enough already, they need to throw in sticky widgets to make it worse.

Medicaid is a taxpayer funded program that has its own rules, such as they are.
Because the taxpayer is paying for care, and memory care costs easily three times as much as Assisted Living does, there must be something from SOME professional saying that this bump up in care is required.

I would take a letter from ALF admin to the PCP and get the permission for this move.
Listen, truth is that we ALL have problems with some of the RULES in life. That's the truth from the moment we are in kindergarten. The trick here is not to wear yourself out with s go-nowhere fight against "rules"; you can't win, and you are dealing with a plate full enough that you just can't afford to heap on more angst. Just take a deep breath, grit your teeth, and go for it. You can put a pillow over your face and SCREAM when you get home. Heart out to you and good luck.
